Coaching Registration Form
In order to ensure that we have the correct contact details and medical information about your child, we ask you to complete a registration form each year - but please do remember to let us know if any important information changes!
The registration form for 2010 can be downloaded here. To make life easier (we hope) just the one form is now required whether your child is attending squads or clubnight.
You should also enrol your child in British Tennis - it's free! See the tab on the left or visit the LTA website for more information:
